Birth certificates for people born in Colorado are available from 1900 to
present. Applications for a
certificate can be obtained from the Fairplay Office at:

The cost is $17.75 for the first copy and $10 for
each additional copy, only cash is accepted. Photo identification is required
along with additional documentation depending on the relationship between the
requester and the registrant. Birth certificates can also be ordered online from
Colorado State Vitals.
Death Certificates
Death certificates for people who
passed away in Park County are available through our Fairplay office. Applications can be obtained from the Fairplay office

copy and $13 for each additional copy of the same certificate. Photo
identification is required along with additional documentation depending on the
relationship between the requester and the deceased.
